I was able to open the pdf file, and yes it looks like I can type
into it before printing, however, for some reason the line for the
film titles cannot be typed into, and also the boxes cannot be checked.

Please confirm that this is the case for you as well, eg. the film
titles and the boxes cannot be typed into. Everything else, yes. I
would like to know if it is me or that this is the case for others as

It brings up the question, will the entry form be rejected because
the boxes are checked by hand, AND the film titles are printed by
hand. Why am I thinking about Nurse Ratchet?????
